With ever increasing demand for health services the NHS is moving towards a Comprehensive Personalised Care Model where the patient has a greater part to play in their long term health and wellbeing. 

Patients with the highest knowledge, skills and confidence had 19% fewer GP appointments and 38% fewer A&E attendances than those with the lowest levels.
